Referrals
If authorization or referral from your Primary Care Physician is required by your insurance plan, we request that you advise our office of this requirement at the time of appointment scheduling.
HMO/Managed Care
If you belong to an HMO, or other managed care network you must bring a referral with you at the time of your visit. Without this referral information we will have to reschedule your appointment.
Auto Accidents
To make an appointment as a result of an automobile accident, you must have a letter from the Auto Insurance Carrier, stating that there is an open claim with the appropriate address for billing, agent contact name, phone number(s), address and claim number if issued. Without this referral information we will have to reschedule your appointment.
Workman’s Compenstation
To make an appointment as a result of a work-related injury, the office visit must be authorized by the Workman’s Compenstation Carrier from the place of your employment. You should have a referral authorization treatment form, address, phone number and contact person name for billing. Include your claim number. Without this referral information we will have to reschedule your appointment.
